I have had this issue before… but super super rare. I think I got it once during the last season, where I could not revive at all.

Yeah, in the 8 years the game’s been out I’ve had it no more than two or three times, I think. That’s why it’s in the known issues thread but remains unsolved, presumably due to how incredibly difficult it is to replicate, and therefore they can’t narrow down the cause.
