However, it seems that 12 refreshes are required! I refreshed 5 times in turn 3 and nothign happened. I lost the game due to this bug

Yepp, same here. The quest progress was like 1/5, 2/5, 3/5, 4/5, 5/5, -1/5, -2/5, 1/5, 2/5, 3/5, 4/5 and finally 5/5 and transformation. First noticed after two refreshes that the tooltip on the hero power said “10 left!”…
Same for me, did not notice that it was bugged until 4 refreshes, then it said -1/5 and started ticking up to 5
