do we need to keep us transmogified each time we change our gear?

Well, transmog isn’t mandatory, so you don’t actually have to do it at all. 
But yes, if you want a new piece transmogged, you have to transmog that piece - you don’t apply the transmog to a slot, but to each piece you put into it.
The cost does add up over time!
You can save preset in your transmog window so even if you replace every possible slot, you can have the same transmog clicking mouse button two times. I have many complete transmogs called “a1, a2, a3 etc” and i’m shuffling between them, depends on my mood, situation etc 
If you want to keep the same appearance, yes.
I didn’t bother during leveling (because the cost does add up) until I started using heirlooms. Now I can use the same gear, with the same transmogs, all the way to max level.
