On launch I get the following error:
Audio device failed to initialize, please check system audio settings and drivers.
Naturally, there is no ingame sound whatsoever.
That happened after a patch some time before Christmas. I hoped a following patch would fix it, but it persists. The problem is not in the Volume mixer, my output devices are working properly (also tested in other Blizzard games like HotS and Hearthstone), and I’ve tried reinstalling my drivers.
I’d appreciate any input as long as I am not forced to reinstall the game or my OS.

Jan of 2025… all of the sudden have the same problem. What did you guys find for your fix?
I tried driver updates, driver uninstall - reinstall.
Audio works for just about everything else, but Diablo 2 Resurrected.
Works for Diablo 3 RoS, and D4.
Had the same problem after i connected my headphones. For me it helped to reset the audio settings in Windows 11:
Settings → System → Sound → Volume mixer → Click on Reset, next to “Reset sound devices and volumes for all apps to the recommended defaults”
