So i just killed Andariel on Hell difficulty with my Hardcore Summoner Necromancer , 1st kill so this is the quest kill and
She drop :
chipped Ruby
chipped Skull
Amethyst
blue Grand Charm (its a 16 life thing so nothing special)
yeallow aka rare Large Shield (so a normal item)
and thats all , 1st quest kill on HELL difficulty
You know the Andy-bug: Kill her, leave map by using the Andy-Portal, talk to Warriv only, enter A2 and make new game… After doin this, the quest-drop is fixed forever 
except that bug was fixed in ressurected… hell ive had times when all she dropped was a stack of bolts and thats WITH doing that
It isn’t fixed. For me she never drops bolts or gold, which is the indicator that she is on her quest-drop-table, not her regular one. And I did exactly as usual to enable the bug.
What’s described in this thread is just bad luck. I had runs where she just dropped two blues, and others where she dropped two unique and two set items.

It is not RNG, currently something must happened. I used to farm Andy quiet a lot, but drops are now very different also with 300 MF. I moved to farm something different.
But is is kind a sad, that first kill can be also without any items.
Yep, I noticed the same, but with cows. It is not RNG, only there is something with the loot table or the mechanism. I made more than 100 cow runs (I know it is not so much, but it can be a base for comparison). Old D2 drops much more high lvl items. I don’t talk about unique/set/rare/etc. Just the D2R drops has much low and very few high lvl items.
Something is not working in D2R as good as in old D2. It must be fixed.
U say the drops are way too low. Some Threads down there, another one is saying drops are way too high in D2R xD
Guys, thats rng. Deal with it. You can be lucky and see “da shiet” dropin’ one day, and the next 2 weeks you only get crap. Same old story since 20 years 
I’m running her with roughly 300 MF too 'cause shes the best option for Tals amulett. Already got some elite uniques and sets, and other useful stuff (Goreriders, Waterwalk, Duriels Shell etc.).
I don’t see any difference to the original.
People are very, very bad at understanding RNG, like its seen from all these topics on D2R. It now makes sense why so many people play lottery

You do know that even if its your first time killing her that there is a chance of a “no drop” occurring its rare but statistical possible. Its how the game works with determining loot. Its all RnG.
A quest kill just “improves” the odds, they don’t guarantee it that you will get a shako and a SoJ…
It’s not weird, just a bad drop. You say you understand RNG, well, that’s it. Just a bad drop.
