So I just did the Ancient quest on Hell but I didn’t get the reward (the +1lvl), is this a common bug and I’m guessing there’s no way to get it now?

You dont get a fixed lvl, you get an xx amount of exp. Maybe you are already at a higher lvl, so the xp werent enough to earn a full lvl

like Skældrig said, usually if you are above lvl 80 when you do it you don’t get a full level, it is a flat amount of xp not affected by diminishing returns found at higher levels.
It is 1.4 million in normal 20 in nightmare and 40 in Hell.
And it is replaced at the end by the amount necessary to gain 1 level if the amount is lesser than the above values.
