Seems your item is bugged then. The (1) in there is the total number of sockets in Path of Diablo. Because you already inserted 3 runes, that can’t be the case. Other mods that show sockets, may show instead the number of opened sockets. And that seemed more likely.
To Danutz23 point, the items shows the runes slots in the right spot. So, that (1) number is clearly wrong anyways.
Positive. The button is not red as it would be with no body near. It is normal and you can see it being pressed. Still says “I can’t do that.”. I have switched to M+KB without moving and I can Revive. This is basically what I do to get minions, quickly switching between controller and mouse; so yeah 100% positive about this.
I just had the exact same problem with Treachery 
Grey Wyrmhide, 3 open slots. Found a Lem rune and started cheering. Now I have a wyrmhide with Sheal + Thul + Lem, but no Treachery…
Not new to the game, been playing D2 since the day the original game came out. Playing D2R, no mods.

It seems, you used a blue/magic armor.
Runewords only works in grey items.

incrdible people put expensive runes in blue item
It was a grey item with 3 sockets, not a blue one. It becamr a blue item after I put the runes in it.

that certainly is weird.
If you put wrong runes into a grey armor, it doesn’t get blue.
Hard to say if you really put it into a blue armor and just want your money back or if it is a bug.
Either way the … at blizz won’t do jack to help anyways gg
There’s a known bug if you make runewords in the stash that they don’t work. I don’t think it has been fixed yet?

put the 3 runes correct in a row and got treachery for my merc.

Look, I just made this post to see if there’s a bug and if other players encounter the same problem. Again, I’m not new to this game, so I know you cannot use a magic/blue item.
And no, I did not post this because I want my money back lol
Just sucks to loose a Lem rune like that and don’t have a treachery for my Trapsin.
By the way, I have made quite some other runewords in my stash without any problems.

Your armor is magic with Artisan prefix.
As Boring has said, You used a plain blue item which gave sockets.
The reason you probably didn’t realise was that there were no other stats on it.

I could have made a mistake of course. Next time I will double check for sure, before I loose any more precious runes
