I recently got the Legacy of Dreams gem and placed the gem into my amulet, but to my surprise it didn’t increase any damage. I then tried to remove all set items from the character and replace them with rare ones, but that didn’t much difference either.
Do I need to be fully equipped with legendary items in order to get it to work?
Only legendary (and ancient and primal ancient) items give any damage bonus. And you cannot have any other set bonuses active (which means that you can use set items but only one, single piece from each).
It’s clearly stated in the description of the gem.

Legendaries is the stuffs you with question marks that you need to identify.
LoD provides 3.75% (per gem rank) increased damage and 2% mitigation for every legendary item you have equipped. At rank 25 those bonuses are doubled for each ancient and primal equipped.
Now major the tradeoff here is that you can not have any set bonuses active. You may use single item from a set but any active set bonus will deactivate the LoD effect.
The damage bonus from the Legacy of Dreams gem (or the Legacy of Nightmares ring set) do not show up on your character sheets as they are a damage multiplier. However, you can still check how much of an effect the gem is having from your character sheet’s details tab…
