What does Squirt's necklace actually do?

It says on it you do extra damage and take extra damage if you’re not taking any damage. I cannot make any sense of the second part of that. What does it mean?

In addition to the details provided by the article Jazz linked, the reason the necklace is used, is when pushing Greater Rifts. Most of the time, you’ll be getting hit a lot, so the necklace never gains any stacks, so isn’t providing any benefit / detriment. However, when you get a Shield Pylon, that gives you an invulnerability shield, that lasts for 1 minute (or 2 minutes if you’re running solo with a follower wearing a Flavour of Time necklace), which means your hero cannot take damage, so the necklace goes up to 10 stacks, providing 100% damage bonus, for the duration of the shielding. This essentially gives you 1 or 2 minutes of double damage.

good points. Some builds -like the monk - have an option to close off any damage.

The monk’s “serenity”: “You are enveloped in a protective shield that absorbs all incoming damage for 3 seconds and grants immunity to all control impairing effects.”

with a rune to double the duration.

That works well with timing maximum stacks, with CofE rotation for the optimum moment of hordes of elites. It’s probably one of the most potent skills in the game

… brings back fond memories of the LoD WoL monk in season 25 - with the fire allies.

Wizard also has different absorb shields to take the damage instead of health, preventing the loss of Squirt stacks. In fact wizard is propably the easiest class to maintain the Squirt stacks.
