When me and my friend were farming Beast in the Ice on torment 3 sometimes the altar would take our Distilled Fear but not summon the boss. To make the altar interactable again, we had to leave the instance and reset, losing our 12 Distilled Fear in the process.
had the same thing happen to me just twice in a row 
my party mates were able to summon just fine, but when i did it, this exact bug happened.
Same here, just now happened one time, I went out and reset the dungeons, came back, done it once, then the bug took another 12 distilled fear from me. 24 lost in total for nothing… I would assume this bug would be done with by now - was the same last season.
Had the same issue. The way i fixed it is to wait till the ice statues reappear before resummoning the boss. Worked for me every time till now
