Summoned Andariel, died during fight - respawned and clicked on mist to re-enter fight and was readmitted to the boss arena, but Andariel did not respawn. Could not leave or town portal, had to quit game and when I logged back on was in Zarbinzet and lost my summon mats.

Same thing just happened to me with Duriel. Died twice, second time i enter the fight, there is one of his spawn alive, but no Duriel, but there is his symbol on the minimap, like he is there
Just few minutes ago I had the same issue. I came back to Andariel after death saw 2 Andariels spawning, died within seconds, came back to ring and there was no Andariel and I couldn’t leave.

Same thing happened to me. And I need to restart the game in order to escape…
