Its not a bug. You get 4 only if you get MW RNG 3 times row to that same Affix. Two times does not raise it to the max. You need 3 lucky strikes + the Michelin Star.
Is it a fun system? Nope.
Yeah it rounds down. With 2 crits at MW12 you have 50% + 45% = 95% increase which is not enough to get +2 ranks, for that you need +100% or more which is only possible with 3 crits.
Apparently it works differently on Legendary items though - Wudijo put a YT video out explaining it.
