Butchered Hero doesn't work

I´ve completed the Butchered Heroes, multiple times now. but the seasonal progress in tier 5, doesn´t complete.

Please assist.

I had the same probelm, completed the Butcher kill on torment 3 for the tier 5 reward, but never got my paragon points
