I accidentally interacted with and found the 10th journal entry out of chronological order before completing the rest of the checklist. Now, when I visit the podium stand in the Imperial Library, entries 1 through 9 are completely readable, unlocked, and white. However, the game fails to auto-trigger the questline completion state.
Because of this sequence break, Journal #10 remains unclickable and permanently locked on the ground in the open world, making it impossible to officially register the 10th entry. As a result, Deckard Cain has no dialogue options and the “Deckard’s Memory” portal to Old Tristram completely refuses to spawn.
I have tried logging in on an alternate character, lowering my World Tiers, and manually playing/skipping every audio log at the pedestal, but my account’s quest state is permanently jammed.
