This is happening every single time.
I think you will find that you disconnected before fast traveling, but it was only apparent when you changed zones (fast travel).
The disconnecting itself is said to be the fault of our ISP’s and not the game which I have verified for myself since release. ALthough there are times when the disconnect has been happening because of the new update where it will quickly load you back in to where you were when the disconnect happened.
I think I might have solved it. I disabled crossplay and cross platform chat and I haven’t had a disconnect since. Fingers crossed.
I tried disabling cross-play and at first it seemed to have fixed the problem but later on the problem came back again. I can’t even do NMD’s now because the chance of losing the key is too high. Correct me if I am wrong but If I pop a key and then teleport to the dungeon and get disconnected, that key is gone? It’s like a gamble playing right now. Are you going to get into a good server or the bad one.
