Distilled Fear on Nightmare LVL 30 or more?

Hello all
Does this item always appear in world level IV dungeons or may it not appear?
There are already a few level 49 ascentral dungeons that do not give Distilled Fear

I’ve finishes more than 20 ND 38-40 lvl and got 0 Distilled Fear.
