In wandering way i dodged an enemy and fell through the floor
I could tp out but when i went back through the tp i ended up back under the floor. Dodging made me fall further down
In not sure if the maps for this area is random or fixed but it was in the top right corner near a ramp down into a semi flooded area with a number of elites.

Yesterday, 4th of July, 2023, my friend and I were in a dungeon where he fell through the floor in a nightmare dungeon area called Quietus Gallery - sorry, I can’t remember which dungeon it belongs too - however, like the OP, he could teleport out and no matter where he went in the area immediately before the door which I had opened, he fell into the floor. I moved on and waited in a quiet area for him to teleport to me so we could carry on, but it’s not an ideal situation to have happen, especially if you’re doing a nightmare dungeon solo and you’re fighting when this type of thing happens.
