Summoning hydras will not proc the familiar enchant unless familiar is on your bars.
It’s a shame to see this bug go to live as it was reported by 4 separate ptr testers between the 4th and 6th of sept.

I can see the same issue for lightning spear as well. I also found a walkaround: Equip the familiar skill and cast it a few times and then unequip it, the issue will no longer happen until leaving the instance.
Still bugged after the latest patch.
