Hello, I just killed myself in hardcore with my barb and I don’t understand how that happened… Can I have an explanation?

There is an issue for me too where i’ve killed 2 of my barbarians by myself. Like literally killed every mob, nothing anywhere - nothing. No mechanics, mobs and i just died and it said “playername killed by playername”.
For now i’m on softcore… :'D
