Lucky hit chance % of affixes on items and skills is indicating on chance for effect of that lucky chance to occur per hit and lucky chance as affix without any effect is used to increase this chance. If you have 15% chance to slow and have 100% to lucky chance than lucky chance to slow is 30% because it was increased by 100% to lucky chance affix.

I think it is just a misleading description, up to 5% means flat 5% chance if lucky hit was rolled.
