Side-quest completion bug

Completed all side-quests in Scotsglen, including item triggered ones. Progress stuck at 46/47. Threads of Envy side-quest seems broken on completion, quest dungeon “Mithering Descent” still accessible with an unkillable mob inside. Likely bugged quest chain or item triggered quest did not count to progress renown.

Completed all side-quests in Dry Steppes, including item triggered ones. Progress stuck at 37/38 despite completing all 39 quests in zone. Likely an item triggered quest did not count to progress renown.

I own the game on multiple platforms (PS5 and PC) and this occurs on all platforms.

Same here. I thought I finished the quest after killing the woman on the tree so I left, but the icon of the dungeon stayed blue so I went back. Mobs respawned, I killed them but the last one takes no dmg, you can attack her and status effects apply but the hp is not dropping.
