Recently I started noticing that even though my frame rate was stated to be 144 the game didn’t feel smooth and playing it made me sick. I checked my settings and realized that v-sync was for some reason turned off. I swapped it back on, but the game would run on 60fps, regardless of weather I set my frame rate cap to display based or manually to 144. I hope there’s a fix to this because I can’t play the game at 60fps or with no v-sync.
Edit: I run the game on borderless because I have 2 monitors.
As far as I know, v-sync caps it at 60fps
Does your monitor support g-sync or freesync?
If yes use these.
(both are better than v-sync while I’m using g-sync.

V-Sync caps the max framerate to the refresh rate of the monitor (video resolution you’ve selected in the overwatch video options).
For example I play in 2560x1440 resolution with fullscreen mode and overwatch lists this resolution 6 times for me with different refresh rates (24, 60, 85, 100, 120, 144). I don’t have multi-monitor setup (I don’t like it) but that along with a borderless setup might make the monitor refresh rate dependent on the windows display settings that are usually set to 60Hz by default which is pretty much enough for desktop applications and watching videos.
Turning on in-game vsync introduces a bit of input latency (usually not that bad) but eliminates tearing. At very high monitor and in-game refresh rates tearing is less noticeable and in those cases some serious players prefer turning off vsync for a little improvement to input latency. G-Sync and Freesync have several advantages over in-game software v-sync, for example less input latency and less prone to stutter when someone has variable FPS or an FPS that isn’t a multiple of the monitor refresh rate.
If you made a mistake and selected a 60Hz resolution in the overwatch menus with vsync off then it made you sick because you suffered both a low monitor refresh rate and tearing that complete each other quite well when it comes to making the gaming experience worse. With a low monitor refresh rate and a much higher game FPS you are making tearing even worse. E.g.: 60Hz monitor refresh rate with 144Hz game refresh rate guarantees 2 horizontal tear lines (with vsync off) and between those you’ll see different frames. If your monitor and game refresh rates are the same then you have 0 or 1 tear line (0 with vsync on).
The game can have a high refresh rate (like 144 FPS) if vsync off even if your display is set up to operate at 60Hz. In this case the game might show 144Hz but your monitor will still refresh only at 60Hz.
