Just died playing hardcore and the passive did not trigger proc.Reason might be cause my Minions where all dead causing it not to proc.

You not aware that the Forums and webpage are in a mess.The link you gave redirects to old forums.And there is no bug report page on new one.And the website support page report function is turned off.First get somebody to fix the web page links and addresses before posting outdated links.And fix the Support page if it contains outdated links.
