so I just realized that all these years I have been gaming @ 60Hz although I have a 100Hz GSync monitor (Asus pg348q).
I turned on my monitor's FPS counter and noticed the game was always capped at 60.
Tried everything to get it to run at 100Hz, including setting gxrefresh in the config and disabling win10 fullscreen optimizations, but nothing would work. Until I tried DX12, which immediately worked.
So my question is: why won't this game run @ >60hz for me in Dx11 mode? I don't want to use DX12 as I have an nvidia card (titan XP)

right click desktop > nvidia settings and then check your monitor hz at resolution options.
and it works fine in DX12 and is much smoother.
in Dx11 the game reports 100+ FPS but my monitor reports 60 Hz/FPS.
no other games have this problem except for hearthstone (sporadically)
Sorry for necro but this problem has popped up again.
All of a sudden I started wow and it wouldn’t run past 60 fps today. Been working great up until now.
Wow Classic runs at 144fps (which is my current monitor refreshrate)
Everything is set to 144hz in Windows + nvidia
No background processes
no framerate cap in game
Reinstalling the nvidia driver fixes the problem for about a day (or until I restart)
Vsync on/off has no effect (vsync should sync to 144hz if on, anyway)
The problem is instantly gone if I swtich to DX 12 (which gives me MUCH worse fps in raids)
— EDIT: I fixed it. It appears to be related to Gsync. If i turn GSync off, i get max FPS in DX 11 again

Okay, I solved it again
I reset my nvidia driver settings and then it worked (despite changing them back to what they were before)
Remove Geforce Experience? It’s about the only thing that would modify game settings behind your back.

You have some weird issue going on somewhere. Maybe related to Windowed Gsync? Or perhaps you need to disable some other piece of software like Discord or Xbox Game Bar.
It certainly does seem like something is resetting your settings somewhere as you mention that the issue gets solved when you reset your Nvidia settings. As to what could be changing it back though, I am not too sure so it will need a bit of detective work.
Like Raijin suggested, external software such as Xbox Game Bar could be causing some funky issues so I would turn this off if you haven’t already. Think about any programs you have which either improve or affect graphical performance. These would be prime targets for forcing a change to your Nvidia settings so turning them off or configuring them may help.
Resetting my settings also turns off G-Sync, which I have narrowed it down to.
It is the combination of two monitors (main at 144hz and secondary at 60hz) with G-Sync.
I have also discovered when I disable and immediately reenable my secondary monitor it will work again for a while.
But its very sporadic. Sometimes it’ll work for weeks, and then it will be back again.
Good to hear it’s been narrowed down and that you have found a way that fixes it without resetting all the settings. Of course it’s not ideal but it would be something outside of our scope to fix. Best to reach out to Nvidia to let them know of the issue if you havent already.
Hmm, I don’t know if Nvidia will help because it only happens in WoW, and only under DX11 in WoW. I think it is too specific.

SOLVED: uninstalling with DDU (Display Driver Uninstaller) fixed this permenantly for me
