I never use that ability, and I am almost level 70.
The abilities I use mainly are Raging Blow and Rampage.
Raging Blow resets so often that Bloodthirst is being used as a filler once every 1000 attacking times, I don’t need a talent point for Bloodthirst to enrage, because the rage meter/bar fills up so quickly.
As bloodthirst is mainly there as a fillerspell when you don’t have RB charges, or as a healing alternative (usually boosted with Enraged Regeneration), what you have noticed do indeed happen and makes sense.
You don’t need or should take the first-hit enrage from BT, as Onslaught will do it for you.
