I boosted a toon to 120. Completed ALL of the nazjatar quests I can, unlocked my heart forge, got my essence and everything else I can think of yet a Level 56 neck is still ievel 390. I am missing the ilevel boosts you get for unlocking the forge (the ones you used to get for CoA rep).
You honestly would of thought they atleast had an intern test the 120 boost.

The problems associated with the 120 Boosts and in this case specifically the Heart of Azeroth are issues we are investigating further as we speak.
I realize it is frustrating to encounter these issues and I appreciate that you took the time to let us know, the more reports we have the easier it is for us to gather that information and make sure that they are investigated further.
If you have not done so already, please take the time to also report this in-game via the “submit bug” option. As I said, the more information our developers have the easier it is for them to prioritize and fix issues.
my heart of azeroth is also broken HoA lvl 53 but ilvl 386

Same for me. Boosted my rogue Billybones on Draenor EU and completed nazjatar quests for necklace unlock and its still extremely low item level. No point to keep playing until this is fixed
I also have this problem on my newly boosted dk

If you have this problem, please make a bug report ingame as suggested & linked by Arzomaju. Posting here won’t get the bug solved, but making a bug report will get it straight to QA/devs in the US to fix it.

Update: Did “In Darkness, I Dream” quest for my minor power on [The Crucible of Flame], didnt get the minor power, the heart of Azertoth is completely broken!
Same issue, sent a bug report.
Level 61 neck, il 400 
Same here, 61lvl and 400ilvl stucked
Same here 62 lvl HoA with 402 ilvl
When is this going to be fixed? Not really going to get invited to group content with a broken necklace being severely gimped both damage wise and ilvl wise.
Any update yet? I’m still waiting for a response on my ticket…
