I just did The Nighthold on heroic, however, I did not loot or recieve eye of Gul’dan, making me unable to progress the quest further.
The quest log says “Normal or higher” difficulty.

Same problem here. Its a bug. Only drops on myth but if your not 740 ilvl good luck getting in group lol.
