How is this acceptable? It’s been like this since off-hands were made visible the first time around.

While i agree it looks rubbish (I just bought that purse, darn it! >_< ), i think it’s because weapons are sheathed cross-body (Main hand item on left hip, off-hand on right hip, while WoW characters are right-handed), and in doing that, where the up-side of the weapon goes is reversed.
That works fine with weapons, where you want the business-end pointing backwards, or items like tomes, where direction doesn’t really matter, but for items where it does matter, like bags, bottles, and items like that, it looks very bad.
