I was doing orb in M+ and entangling spawned while soaking orb. Suddenly orb disapperas. Then next 2 orbs didnt spawn so only 1 and we didnt get first debuff coz 2 orbs didnt spawn. 
Got logs or proof, because based on history your statements are almost never correct.
