For some reason it is not dropping. I emptied the room with another DK a few times and neither of us got the drop.
It’s supposed to be 48% drop rate so it must be a bug that it isn’t dropping for us at level 50.

Thats true. Spend over half hours yesterday farming: didn’t drop from single one. It might take a while before they fix such a minor thing.
Same here, cleared the cave a few times, no drop
Do fill in a bug report in game, you wont get a response but it will alert the relevant people that there is an issue.
I have the exact same problem. I have used in game report and contacted GM about it and GM said that there are no known issues that they aware of. So I am hoping they will fix it quickly before DFA is gone.
