Each Classic server will become a TBC-Server AND an Era-Server and keep its name in both versions. There is a chance that Era-Servers will be connected, though.

I may be wrong about this so I’ll edit my post if so.
I believe that all current Classic servers will become TBC Classic servers. However, at that point, each realm will be “snapshotted”, creating a Classic only version of that server. So there will be both a Hydraxian Waterlords Classic and a Hydraxian Waterlords TBC (for example).
When we next log onto a character, they will be given a choice to advance to TBC or remain on Classic era. I believe there will also be a paid option to clone the character, keeping him on both servers.
There will be restrictions of course to prevent exploits (we hope!).
This is a good question, it feels like there will be too many classic era servers for the remaining population if current servers get copied? I just suspect a majority will move on.

Actually, your question is wrong. There are no dedicated servers anymore. Now WOW run on cloud servers so there is no dedicated machine that you could recognize this machine is this realm. But anyway, if you talk about realms individually then yet, there will be a TBC realm per every current Classic realm. For example for the current Pyrewood Village Classic realm when pre-patch launch it will be a Pyrewood Village Classic forever realm and another Pyrewood Village TBC realm, but there is not a Pyrewood Village dedicated server. Realms and servers are no longer the same thing.
Blizzard have said that once the population of Classic era realms are settle they will connect those realms if needed, what essentially means the number of realms will be reduced.
I don’t remember seeing that anywhere.

Here’s how it works…. Existing realms will patch with the new Burning Crusade Classic content. If you choose to experience all of the new content, everything for your character remains the same including your realm, guild, and more — but you’ll have access to the Burning Crusade Classic and all the updates that come with it.
Realm names will remain the same for both Burning Crusade Classic and Classic Era realms and player characters will exist as they are before the pre-expansion patch on both era types.
All it takes is a little google search to get the info you need.

This question is about names reserved for characters copied to Classic and TBC but at the end the developer said:
“One of the things we are also concerned about thought is what that does look like in Classic Era like as we expect most players go to Burning Crusade and we’ll probably have to do something to resolve that; our current plan is that you’ll be able to keep your name but probably have something similar to our Connect Realms that let you have your realm name at the end”
