I completed the Horrfic vision of stormwind with all objectives cleared, but the mask of the multitude did not drop. This item should have a 100% drop rate upon full clear or im wrong . please investigate the potential issue.
The Faceless Mask of Multitudes drops when you have cleared every MOB, not just every objective.
There are a bunch of comments here from people listing which mobs they killed and may not have killed. Talks about mailboxes and trashpiles and stealth mobs. Some of them may help.

you have to kill all adds , but you dont need to open mailboxes and etc .
