Its not a drop, can be obtained from the vendor on floor 3/5 (it doesn’t always show up and is completely random)

Yes, the vendor is RNG. You have to try a few runs and then save the souls for buying the transmog.
God, SL was so bad in its grind.
Do I need to flawless the run, like get 100 on each floor to get a chance at a vendor for it to spawn?

It’s a wandering vendor. Sometimes the vendor is there, sometimes not. It also depends on the difficulty level. You have higher chances at higher levels as far as I remember.
do it in level 12 and above and flawless is not needed 
it will not appear in levels lower than 12.

Weapons come from floor 3/5 and cost 300 each, backpieces come from flawless vendor at a cost of 1000 each.
