The Blood Death Knight Darkspear troll with white skin and red tattoo’s are the closest you’d get to a blood troll.

You already play a race that attacks kids for a beginner quest.

This is one of the minuses in new starting quests. So Ok, now you can make Night Elf with blond hair + yellow eyes + white skin (Highborne). You appear as a rookie on a ship with Alliance expedition (ok, Highbornes joined Alliance in Cata).
So, you can officially call yourself as Highborne elf, not a Night Elf. Since according to a quests, you never was in Darnassus e.t.c.
But, when you fly to the Stormwind, in the last quests of the starting chain, background voice ruing it all. It says that you have left your sacred forests to help Alliance and to defend the nature…
So, you are not Highborne because of that background voice!
And each race has that description in main faction city. No matter how you will customize your character, you will be still hardcoded race, from classic, and you cannot RP different clan/tribe/nation e.t.c…
I mean, you can still RP as whatever you like, but I agree that they could have done a better job with the race descriptions, especially in regard of the new customization options.
Like for trolls you get this: “Renouncing the savagery of the other tribes, you and your fellow Darkspear trolls protect Azeroth with cunning ferocity.”

Yes the lore is really static in WoW, not really ambiguous like a sandbox game would be. People pretend it’s sandbox to escape the archetypes but there will always be immersion breaking moments like that.

Sadly it won’t happen because they are just an “evil quest NPC” race and lorewise they are sworn enemies of the Zandalari Empire (thus the Horde will never accept them )
Luckily the playable Darkspear trolls received these new bodypaint options so I can give my DK a kinda blood trollish look

Danuser shot these sort of possibilities down, when he clarified none of these customisations were to be considered as canon.
You can still make you own headcanon with visuals to go with it. Although you’ll have to accept that your character isn’t part of the Horde in any canon way (only Darkspear and Zandalari are).
But Trolls do have some cool red tattoos that can be combined with pale skin and ragged armour pieces.
You can always play as that one Blood troll or any other who decided to join the Horde.
Unless we have new backing for it (which we don’t), the last official stance both Horde and Zandalari had regarding them, was a “kill on sight” policy.
Any iteration from it hinges on players imagination alone.
