If you fail the quest by dying do you loose the orb. ?

If the demon Elite kills you, abandon the quest, then speak to Tabetha again.
You do not need to obtain another orb. I spent a good couple of hours trying to get another orb to drop before I simply decided to abandon the quest and go back to Tabetha. Lo and behold, she gives it to you again …
Though, once you accept, she conjures the demon again straight away. So make sure you have some buddies along to help you!
