Okay… I’ve noticed that there’s no consistant pattern when it comes to the bloodhoof guys casting fel surge.
Mostly - he’ll use it right after earthquake, which is nice because the indicator is just fine and long enough to setup your mindbender and fade. The second cast is easily neglated by fearing during the intermission (when the worm is not crawling through the ground). However, later when you’re close to pushing him ~ 20% ~ sometimes he’ll cast it when the worm is still digging around. You can catch it with the fear, fine, but when he casts it again in the intermission, he’ll oneshot you (i’ve tried barrier+PS+desperate prayer). Unfortunatly, this is not always happening - it seems to be completely RNG which makes the thingy inconsistant! Well im running with SL gear and no consumables and making it harder for me anyways but like I have absolutely no issues with the mechanic but it sucks when you stuck in the situation that you cannot do anything! I wonder if this is intended, as some kind of a soft enrage? (happened to me when both had 5% -.-)
Ive done quite a lot of tries as disc, and Im pretty sure he never casts it twice in the same intermission. You should be able to fear all of them.
As for oneshotting or not, the combination of full hp, masochism, shield and mind blast makes a very big difference. You can survive the casts for a loong time without using any big cds.
You can also survive the sonic scream from the worm after tugar is dead, with proper cd management: once with PS, then with barrier+rapture/desperate prayer. And dont forget (like I did) that you can fear his casts. So you should be fine with worm at 25% and possibly higher when tugar dies.
It obviously depends on your gear. You might wanna try out with more vers. Base ilvl honor gear is quite cheap, and you’ll prob find some reasonably cheap stuff on AH too.
Ive done it yesterday you may want to get northrend engineering 5 for the tinker belt interrupt it lines up perfectly and you can take solace for extra damage instead of mindbender
if u use it on cooldown and at the first second he starts casting fel burst it lines up perfectly while fearing the other cast and i agree with trip the cast is always consistent you might want to download tugar bloodtotem weakaura’s that could be very helpfull
also the easiest way ive found is to kill them both at roughly the same time while killing the worm first while tugar is around ~5k hp

Okay, so I did it without any abuse and even without consumes so I can tell you how you do it.
You need to sync it so he casts one time where he+worm are up, and one right after earthquake. Sometimes he falls out of sync but you can kind of expect this behaviour. It happens sometimes that the worm is under ground but the bost casts his fel thing. For this you need to tank it without using fear. You need to be topped and use defenses. I usually used pain sup together with rapture and it helps a lot to bring it back into sync.
If you see it going out of sync again, for the next one you need to desperate prayer+barrier+ mind blast beforehand.
