hello, so in my 2 last hc+, we all greeded and needed on the items, but it didn’t show the rolls/who received some of them. it simply shows that people either greeded or needed in the chat, but nothing further. this is only for some of the items, one that I noticed in both instances was the Frozen Orb. is this a visual bug which only i have, or did the loot just not get distributed? i did not receive anything in the mail, but then again, i don’t know if i won the rolls or not since they were not shown in my chat
You got scammed. Rolls and results show up after everyone clicks something. Some people are desperate enough for that frozen orb that they don’t click anything in hope that someone will hearthstone out before timer is over which would increase their odds.

Also, if someone experiences a loading screen before picking need/greed/pass, the item will despawn and nobody will get it.

If you don’t stay in the instance until the things have been rolled out, then you can’t get the items.
Just make sure you don’t leave before the roll 
Someone just didn’t click anything at all, in hopes of you leaving the instance before his roll timer runs out. Then the item defaults to whoever is left in the instance.
Just wait patiently, check the names in the chat and call out the player whose name is missing in the rolls. He will of course pretend to be afk, or not see the roll window (this can happen, he just needs to /reload ui).
In 4 years of running dungeons almost daily, I have never ever seen this happen. If someone leaves with a loading screen, they just auto pass when the timer runs out in 3 min. If, however, someone rolls need, then leaves, he may still win the item and it will remain on boss, inaccessible to anyone else, until such time he comes back to loot it (which most wont, so the item just despawns with the boss in an hour). If someone experiences a loading screen as the boss dies, he may be eligible to the item but not see a roll window, like I said above a simple reload helps. Worst case that ever happened to me was a loot master logging off in a bout of rage before distributing the items in a Kara pug. Then they were effectivelly ninjaed to “no one” as the boss simply despawned in an hour (yes we waited to see if he logs back in to steal the items, but he didn’t).
Well, it happened twice to me. Maybe it’s not a 100% occurence. The chat did not show the rolls, the /loot window did not show the item ever getting rolled for, but the corpse could not be opened as if no more items existed on it, made sure to wait over 5 minutes each time.
Do /loot and you’ll be able to see detailed roll info.
There is a bug where sometimes items don’t appear to some people and they have to /reload their UI to be able to roll on it.
That’s what I’m saying, the /loot window did not show the rolls, just kept showing who picked which option, with “–” by one person’s name forever.
