Off Hand vs Main Hand

Hi,

Any difference in Main Hand / off Hand or not ? Are some weapon better than other on Main Hand ? Should I have my highest ilvl one on main hand or it does not matter ?

Most likely you have to use highest ilvl on main.
To check if indeed it does make a difference equip both weapons, check tooltip damage number while out of combat and unbuffed, then swap weapons and check again.
