Orta, the Broken Mountain fails but shows as completed

I did the WQ killing Orta. A nice raid group, boss dies, shows as 1/1 killed in questlog but shows up as failed. Anyone else having this issue?

So, if this happens to anyone else, relogging seems to fix it.
