Well, from what I have seen on here, my rarest item is my Headless Horseman’s mount, which is the second drop of it for me. It originally dropped on a character I then deleted, and it was before the time of Account wide mounts, so I lost it.
I have no idea how I’d even work out what is rare or not.
I’m going to go with I’m a pleb who just has regular stuff other people do

But are those really rare? Half of them I have it too, and I’m not a rare collector.

oh and I have the ravager from OG scarlet monastery still fun to get out for that spinning proc time to time

I got the SC2 and D3 ones, plus about 3 of the wow ones. Still look at them occasionally
i have this toy from back in vanilla which turns you into like a spacegnome…
it came from a leaper gnome event way back when.
also have a old pvp title on my vanilla rogue.
on horde the same toy turns you into a darkspear troll.

I’m not sure what the rarest item I have is, but I do still have in my bags, a couple of potions of luck from MOP… They’re greyed out now and it says on the tool tip something like “Not so lucky after all”

Certainly not all of it, most of it is just old, but I have no real way to gauge whether or not it is rare as well (some of them are according to that website some people used). The achievements are all rare, either way.
I had that but deleted the character it was on. I’ve lost so many toys because I destroyed them due to needing the bank space at some point.
Some I can get again but others are gone forever.
I have the 10k polished pet charm pet. Kinda unique for me since the farm was pretty annoying and exhausting.
The absolutely rarest thing I have is the X-45 Heartbreaker (Big Love Rocket). I also have the Time-Lost Proto-Drake, and I feel so bad about both because I didn’t even try… and I’m a druid and never use mounts anyway 
Apparently, The Exodar Elekk from the Argent Tournament is actually rarer than the Time-Lost, though… who’d have guessed?
My favourite rare thing is my beloved Windrider Cub - got it for my birthday in, uh… 2011, I believe? It’s still coming along on almost all my adventures 
Rarest non-TCG pet is Frosty - WotLK Collector’s Edition pet.
Other than that, it’s various TCG-items I bought for a few dollars just for fun way back when, and they now sell for more gold/money than I ever had (if only I could sell them).
I don’t even wear the tabards because I don’t want to look like one who spent millions for them on the Black Market 
I did sell one of the pets, though - for half a million gold. That was a LOT of money at the time!
I was about to say “my dignity”… But then I read “in the game” which would be Teebus Firedagger or however the HD rework is called.

I dont remember exactly when but it was around end of cataclysm or in MoP… I was about to make a run to Karazhan for transmogs and general gold etc.
My friend and his girlfriend just started playing and they were just hit 70… so I offered them to carry them in raid to get some xp and maybee some item drops.
First boss and mount drops!! Needless to say, it was the very first time I see it drop… even if I was raiding karazhan in TBC and did it numerous time since then.
My friend tell me to pass it to me since I boost them there, but I want to be cool and such and tell them to roll. His girlfriend wins the mount… didnt say anything but I felt sad cause I really liked that mount.
Next week, I am running Karazhan again, this time alone. Kill the boss, mount drops again!!! and that was the super rare moment in wow… seeing that rare mount drop 2 times in 2 weeks!!!
As far as rarity goes, It’s kind of “up there” but nothing like “omg wtf top 0.1% of rareness”, except for A tribute to Immortality, which is definitely the rarest thing on the list. You don’t see that too often, even moreso if you also have one of the removed weapons from that raid.
Idk about the other achievements though… I do see “The Immortal” title quite often (or rather what’s considered “often” amongst rare titles)
I can tell you for a fact your rarest “items” are the blacksmithing patterns for the frost resist gear from Naxx, since there’s very few crafters of that stuff still playing and twinks in TBC didn’t tend to collect the crafting patterns. The gear and enchants yes, but not the patterns usually, so… those are far rarer than the actual drops from old Naxxramas.
Obviously achievement-wise the “Realm firsts” are very rare as well since… well… there can only be one “realm first”, so you don’t see them too often. Legit ones, anyway. There’s a bunch of players around who duped their achievements so they kind of pollute the pool sadly.
Tier 0.5 itself I’m not sure since a bunch of Twinks have it usually, but the fact you still have the summoning items themselves adds to the rareness, so… that’s probably the third or fourth rarest thing you have, either before or after the S7 and s9 Elite weapons since I’m not sure how many people did rated arena back then but I recon it’s more than those who kept the 0.5 summoning items.
Ironically what’s rarer than for example, Arcanite ripper or drakefire amulet, are the random rubbish items such as Faded Photograph, since most people tend to have just discarded those after having done the quests.
Probably Invincible. I don’t have old, rare novelty items tucked away.
