Given that realm first titles were removed in later expansions, along with the change in philosophy in classic such that generally desirable changes might be considered in a limited context, might a change in the way that realm first titles are awarded be warranted?
For example: highly competitive guilds are likely to change comps for some bosses. Both Yogg+0 and Algalon progression comps exemplify this.

No, you do it first and you get the realm/world first reward, doing it second is nowhere near the same, it’s still a feat but not one you should get a reward for.
I am surprised they kept them all in to be honest in the first place, given that they want to discourage that kind of play, most of them are fairly stupid to begin with. The realm first herbalist was claimed about 2 minutes after WOTLK Classic went live because a druid had somehow managed to get to Borean Tundra an hour before launch, and was flying around Sholozar after that.
Back in real WOTLK, the realm first Obsidian Slayer on my realm was claimed by a PuG that wasn’t even a full raid.
On top of all this, in this classic iteration of the game, realms have considerably less of a community than they did back in the day, so yeah I agree they are pointless and should have not made it into WOTLK Classic in my opinion.
Technically, if the realm is dead enough it should still be possible.
I don’t mind Realm Firsts being there. Not gonna get any, but it doesn’t hurt me either. Had one back in the day and it was pretty cool, but people just played slower back then. No matter how “Classic” you get, you can’t change people to how they behaved 15 years ago.
