I’ve run twice on two different chars, both defiantly set to Mythic, both times the image of Arcanagos killed image of Medivh, no boss fight started, so nothing to kill for Smoldering Ember Wyrm. No longer in the adventure guide either which I am sure it was before.
Anyone else run kara and have this issue?
Last week it was fine.
I’m newly back to wow after a 7 year break, so this mount is new to me.

It hasn’t been intentionally removed, so it could be a bug. Assuming you set the dungeon to mythic, did the crystal collecting in the time-limit, and everything else prior to the fight properly, there shouldn’t be a reason for it not to spawn. You could try and reset the instance by leaving it and right clicking your portrait and then selecting “reset instances”, though I’m not sure what that does in regard to the event itself.
But since you seem to have done this before successfully based on your post, I’ll assume that you did do it right. In this case, it would be a bug, but whether the bug is something where it fails to spawn once or at all, is up to you to determine. Assuming you get another try at spawning him and it still doesn’t happen, open a ticket.
I have had the same situation right now, Medivh died immediately after the conversation was started. Will write the tiket, hope it helps
