Been phased away from active hunts i was doing and other stuff completely arbitrarily multiple times now without joining or leaving any groups or anything that would explain it. “Best” experience so far was getting phased while doing a pet battle which cancelled the battle.

Totally agree. Random phasing has become such an issue lately. A lot of people got phased out of a Community Feast yesterday that was only 300 away from Legendary and suddenly we were doing an Uncommon one.
This exact thing happened to me earlier today - was all the way up to the 6th level of legendary and phased into a rare.
Absolutely this.
I could be rarely mining in the Cavern without bots when after a few minutes i get phased into the area with tons of mining bots.
One moment you are fighting a rare and the next it despawns in front of you and other mobs re-appear, meaning you got phased mid combat.
