Odd, never happent to me. Either way i suggest trying to check your mail if you got any, if you don’t you should probably try opening a ticket

The drop should be garuanteed.
I haven’t experienced this issue myself on any off my chars, but I can understand it must be frustrating!
Did you also try checking the inventory off your alt characters? Maybe it dropped and got lost in the clutter off SL items.
Otherwise I’d suggest you to try Level 6.
Somehow somewhere in my memory I remember something similar and I fixed it by going higher layers.
