I don’t know how many items there are that change your visual appearance, but I do know that many druids would love to be able to use the visual appearance from the Fandral’s Flamescythe.
As for Fandral’s Flamescythe, it’s not the only way to get fire kitty look. In this guide I listed all the options to do so and it includes consumable items you find in Firelands and the toy that drops from the boss.
I’m not aware of any other items that compltely change you character’s look if you equip them.
