Unable to upgrade weapon and not getting Harbinger of the Runed achievement

I have a 636 two-handed crafted weapon and I am not getting the discounts for crests on any other two-handed weapons. Also the achievement for getting everything 619 or higher (Harbinger of the Runed) is not popping for me although everything is at 619 or higher.
