They have always existed.
Only in the past they never had a label, you simply knew it as
“Hmm that item has a low drop chance”. At least now its labelled as such.
Weren’t they mainly cosmetic in the past?
On certain encounters sure, but not in the overall sense like in DF.

I’m not a fan of the very rare items esp as they are the ones everyone is usually after but no one should be fired for it.
I never understood why people say anyone should be fired. Sure the idea might not be to your taste, but to say that a person should lose their income and potentially their home over a video game is an extreme statement.
Thats the reality of life. People lose their jobs for bad decisions always. We are paying his salary.
I agree with the OP , this is a bad game design, creating an item wanted by everyone with a very small drop chance , creates frustration beyond belief and frustration doesnt equal more sales, on the contrary. When people have enough sh*t, they move on.
I don’t disagree that loot rarity is a frustration, but that doesn’t mean a person should be fired. Perhaps if they were ordered to fix the problem, and they refused to do so, that would be another matter.
There is a private server
Every raid/dungeon since beginning of wow have had different drop chances for items, and some have always been far lower drop chance than others.
Obviously, you have the mounts or legendaries with very low drop chance, but also certain weapons or trinkets have historically dropped far less than other items.
Classic example would be something like Felstriker from Rend Blackhand
