Let’s say i did all campaign quests up until this point on one character, which counts as account wide, but i can still do the campaign quests again on my alts if i want. Let’s say then then I decide to play a different alt for a while. On this alt I have leveled doing side quests only and havent touched the campaign…
… Now a new patch is released with more campaign stuff, will I have to go back on this alt and do the campaign quests from release of the expansion before i can do the new patch campaign quests?
No,
Even before Warbands, Blizzard gave us catch-up campaign progress on alts.
In DF, if you had opened Forbidden Reach or Loamm or Emerald Dream on one character, your others would get a skip option.
In TWW, when my second character dinged 80 from side-quests and Delves and Dungeons, he was immediately given a choice by Brann in Dornegal to skip the levelling campaign and start the current patch campaign.

Yes. See Brann in Dornegal for that,
