Isn’t WoW designated 16+ of recent years? Plus it’s cartoon appearance would most likely be making it okay. Think of all the cartoon shows that you watched as a kid that would sometimes show someones brain popping out etc.

It generally isn’t a large issue if the gore on display is cartoony, see abominations, which are far greater offenders in that regard. An exposed brain like on that mob is even less like gore and more like an image from a science textbook.
Wow’s only rated T by the ESRB (North America) as far as I can tell, PEGI (Europe) still has it rated as 12+.

Lord of the Rings and the Hobbit (Battle of the 5 armies included) got a PG-13 rating for example because it depicts intense fantasy action violence and frightening images, despite how Peter Jackson went crazy with Del Toro’s monster designs in the Hobbit movie, some of which was basically nightmare fuel that you’d expect to see in Maldraxxus in this game

Having your game go up an age rating also actually takes some more drastic stuff, usually. As the rating agency has to actively care about what you’re doing with it. They often tend to hand out a rating and then just keep it.
Basically the easiest way to up your age rating is if you suddenly decided to include a lot of sex or drugs. European rating systems are a bit more sensitive when it’s about gore too, but that’s been getting less severe over the years.
Oh, and some countries will get a bit hissy about you mentioning same sex relationships. No matter what.

Depends entirely on anyone bringing it to attention to the people who care. There aren’t that many that are very invested in actually following along with side stories in video games.

Might be the point of Nathanos… But while it’s fine to like him as it is, it’s just as valid to detest him for that.
I’ve attended plenty of RP-PvP events that have the players talking exactly like Nathanos does. The same smug, superior, self-satisfied tones. It’s not a ‘Stormwind’ exclusive thing even slightly.
To be honest I find Nathanos to be quite a nice change of pace since literally every NPC loves the player for being the Champion of Azeroth and Alliance/Horde so it is nice change of pace of him being snarky and slightly annoyed at our accomplishments.
