They immediately sap any stealthed player that comes in range - quicker than just using a macro for it. Are these running a script? It seems they have impossibly super fast reactions. I dunno. And yes, I do suck but still - seems very fishy.
Yeah, anyone who spots you first in a rogue v rogue duel and gets the opener or sap off first is obviously using a bot or a script.
There’s no other way.

I’ve noticed this too on my hunter. I can be hidden in shadowmeld when a rogue comes by to hand in a quest, fire off serpent sting first and they insta-vanish to dodge it??? Either they have the fastest reflexes in the world and always prepared or they clearly cheating.

I was using a nearest distance macro in retail but i didnt try it in classic with my rogue so i dont know if it works in classic.
Retail sap spam macro was something like this:
#showtooltip Sap
/console targetNearestDistance 15.000000
/targetenemy [noharm][dead]
/console targetNearestDistance 41.000000
/cast [harm,nodead] Sap
you just walk while spamming your macros then you’ll be "oh i sapped A rogue) you sap them without noticing xD
the power of macros is better than magic!
you can even blind/CS players without targeting them.
